Not really sure what to do, a buyer asked to return an item and from their eventual reply I got the impression they’d bought it without reading my listing properly. I had listed it as no returns and because of their unhelpful attitude I stuck with my decision and refused the return request. But it looks like they’ve sent it back to me regardless.

If buyer opened item not as described case, you have accept.
You can refuse change of mind return as private seller.
If no case and buyer just returned item, ebay won't get involved.

The item was exactly as described though, which is why I refused the return request. And they sent it back before a decision had been made on the return request.

What reason did they give when they opened the return?
Did they say the item was 'not as described'?
If they open a case, your choices are only to message the buyer, refund or accept the return and supply a label.
What has happened so far?
Have you actually received the item back?

If it was an ordinary return request, as a private seller you didn't have to acept it. eBay should support you and if they do you will be fully entitled to ask the buyer to pay for the postage if they want it back again.
If they opened a money back guarantee case eBay will support them and provided they entered the tracking number to prove that they returned the item to you you will be required to refund them.

So the reason they gave when they opened the request was “does not fit”. For context it was a camera for a tv, that only works with certain models of TV. They claimed that the item I sold was incompatible even though I specified exactly which tv was needed for it to work. EBay gave me about 3 days to decide if I accepted the request or not. So after multiple unanswered messages and one message where they claimed I’d lied about the product I chose to refuse. And then today the item turns up in the post.

If they opened a money back guarantee case would I have any opportunity to try and prove what I sent them was exactly what I said it was?

As they chose the reason 'doesn't fit', that does count as a change of mind and you would be in your rights to refuse the return.
However, I would advise you to refund them.
If the item is in a condition where you can sell it again, and the buyer has sent it back at their own expense, personally, I would refund them.
That will stop any further issues.
Block them so they can't buy from you again.

They can't open another case.
Why waste your time and energy trying to prove you were in the right.
Who would you want to prove that to? How would you prove it?
If the answer is 'Ebay' - they will never see the item sold.
That is pointless, life is too short etc.
Just refund them.
