Sold item ebayer didn’t want it went through eBay which gave him refund and asked him to send it back but not received and been 3 weeks what can I do

If buyer opened returns case, you should have issued returns label and only refund once returned. 

If you refund 1st, ebay won't make buyer return item.

As a seller you should be aware that sellers will always will lose and that buyer protection takes precedence. Failing to respond to this sort of case, ebay assume that seller does not  want item returned, and refund at sellers expense, plus seller gets a defect, which can be very damaging for a seller.

There is also the risk of negative feedback.

This was a change of mind return, so the buyer should have sent you a return request, through eBay. If you confirmed your accepyance the buyer would have been responsible for returning the item before being refunded. On receipt of tracking proof of delivery eBay would have issued their refund.

What went wrong with this procedure? The bad news is that if you refunded the buyer before receiving the item back, that will be the end of it. eBay will treat the mattr as closed.
