I sold an item on ebay. The buyer said it was a fake and wants for a refund and doesnt want me to invole ebay in this process. He says I will get a negative review. I asked him to return the item for a full refund but he is sitting on the fence. He is bullying me in saying I will get a negative review if I don't allow him to keep the item while he gets a full refund as well. What should I do?

Do not engage with him. You have told him to return the item. He knows the score.
Let him rant on.
You can't report him until he does something to report for, which he hasn't yet.
The ball is in his court now.
He only gets a refund if he opens a case.

You have an abusive buyer who has already contravened eBay policy.
He is all too obviously attempting to extort, by threats of his leaving negative feedback, a refund outwith the normal means of the return/refund process.
I would advise reporting this behaviour now and allow eBay to protect you... they may even decide to contact him and warn him off on account of his bullying.

I had something similar happen to me...sent the item and there was an extra long delay between sending and it arriving. Then the buyer claimed that it had been delivered several houses away and RM pictures confirmed this! In the end I just let them keep it as too much hassle for the amount of money. Fortunately this type of activity is the exception as most buyers are genuine.

Here is a link to report buyer, if for any reason link does not work, go to top of page marked "Help & Contact" and type in search bar "report a buyer" and info will come up.

Also if messages have been sent through ebay from buyer threatening negative feedback if unable to get refund and keep item and not involve the correct procedure of opening a case for refund, ebay have access to these message and can be used in your defence.
