My buyer has requested a refund as item hasnt arrived, if i accept the request do i lose out? Seems a bit unfair?

With Simple Delivery (assuming you used it) you are protected if the item is lost/damaged in transit. That means ebay will pay the refund, not you, when the buyer opens a claim. So if the buyer has opened a "not received" case eBay will issue the refund themselves and you keep the original payment.

Did you buy postage or did you use the label the buyer paid for using Ebay Simple Delivery?
If you used the label provided by Ebay, not postage you bought yourself either from Ebay or elsewhere, then usually if the buyer opens a case stating the item has not arrived, Ebay deal with the claim anyway.
Has the buyer opened a case through Ebay? If they have, you will get an email from Ebay rather than a message from the buyer.
