Has anyone had a buyer accept item, leave glowing positive feedback then 5 days later want to return item as not as described - she’s raised a returns and eBay saying I have to refund?

Yes you will have to Refund. Buyers are covered for 30 days by the Ebay Money Back Guarantee. If you don't comply EBay will Refund from your PayPal account and you will receive a Damaging Defect on your Account. Best to just Accept and generate the Returns Label through the Case. Your EBay Final Value Fees will be Credited back to your Seller Account.
Although it may seem frustrating the best option is to take the advice that you were given earlier and issue the buyer with an eBay returns label. Once you receive the item back again refund the buyer's money via eBay, as that way you will not pick up a defect in relation to the transaction, so long as you refund swiftly as opposed to keeping the buyer waiting for so long that he has to ask eBay to step in and force the refund. If eBay have to get involved and force the refund, or if you make the mistake of issuing the refund via PayPal rather than through the case that has been opened on eBay, then your account would be hit with a defect as a result. Therefore, it's important to get it right - issue the refund voluntarily via the eBay case and your account will be unscathed by the case that the buyer opened.
Once you've received the item back again if the item is still in a decent condition and is good enough to offer for sale again then you can relist it for sale a second time should you wish to do so. However, if you have not already done so then make sure that you add the buyer's User ID to your Blocked Bidders List.
