if an item doesn't sell, and you list it again, do you pay the buyer protection fee each time?

As a seller, you don't pay the fee.
The fee is paid by the buyer and retained by Ebay when you sell your item.

The biggest difference being, the buyer pays this fee, not the seller as before, with the selling fee.

Generally, if the item is sold for over £5, it is a lower fee and it is paid by the buyer, not the seller.
Old system fees were about 13% + 30p so, on a £6 item, you would have paid 78p + 30p = £1.08
Now, a buyer would pay 4% + 75p so, 24p + 75p = 99p
Plus, Ebay also collected 13% of the postage fee from the seller. Buyers do not have to pay an additional 4% on the postage the seller charges.
It does make low cost items more expensive.
It's another change that sellers and buyers have to adapt to.

Yes! UK-based private sellers pay no transaction fees for all categories, except Vehicles, and receive 300 free listings per month. The Buyer Protection fee is paid by the buyer to eBay. So if you list an item for £10, you'll receive £10 when the item sells.
