I refunded a buyer as the item didn't arrive, now the item has arrived and she wants to keep it, how does she pay for through ebay

I'm afraid it's not possible for the buyer to pay via ebay,so you both need to agree on a payment method outside of  ebay so by bank transafer,cheque,paypal etc.
