A buyer has purchased 10 items from me using simple delivery, however it has charged them postage on each item (£27.50 total postage charge) they have messaged me asking for a refund of additional postage charge and I can’t refund it due to simple delivery. How can I get a refund to them?

If you are sending the items together to combine postage eBay will automatically issue a refund to the buyer for the unused labels. The money takes about 30 days to get there though.

The thing is it’s 1 label, but they have paid the postage charge of £27.50 and have asked for a refund

if the postage has already been combined by ebay and that is the charged that came out - and looking at the screenshot - that does seem to be the case, then there isn't anything you can do.
They'll have to complain to ebay directly.

They saw the postage when they paid and entered into the contract, if you refund the difference the next thing will be they say they only got one item and want a refund for the other 9 items.

There isn't any "difference". Just one expensive Simple Delivery label. @tf723ll1ot can politely tell the buyer, that the postage cost is set by and has gone to ebay, and they can discuss with ebay if they want to.
