I've just sold an item with postae as an option. The buyer has paid for postage but I am now concerned that the item is too fragile to be sent by post.. As the buyer is a reasonable distance from me I am happy to deliver it in. person. How does the buyer confirm receipt of the item so that i then get paid?

The buyer can't mark the item as received.
The best way to do this is to cancel the sale and relist the item as collection only.
The original buyer could purchase the item again and give you the collection code when you deliver it.

You would get paid eventually, eg after 14 days, however you leave yourself open that if you deliver by hand and you have an unscrupulous buyer they could then open an item not received via ebay as there is no proof that the item was sent by post, they would keep the item and get a full refund. You don't want this.
As @jckl1957 if you don't want to post and deliver by hand, message the buyer, tell them what you are going to do, and say that you will knock the postage off saving them money and relist the item as a BIN for them advising them of the item number so they can buy it again.
