I cannot answer the first part of your question but for the above, when an offer is accepted, it will still stay for sale until the buyer pays you.
Must admit, several times when this non payment happened to me, I have sent a polite message to say offer is accepted and please can they click on the Pay Now alongside their item in Purchase History. I've always had a reply thanking me, as they didn't know the transaction had not been completed with a payment.
Needless to say, if they are a non payer, the item is still on for sale for others to see and buy, and you just block the time waster after 4 days of non payment from returning to your sales.
