I was watching an item and the seller has made an offer, which gives me the options to accept or decline.
The item is also eligible for the Perks15 code. However, if I accept the offer (which it says then commits me to buying it), would I still be able to apply the Perks code at checkout and so get 15% off the offer price, rather than the original price?

Who is making this offer, Ebay?
If you have a valid offer, why not read the T & C, they will answer all your questions.
I have never seen such an offer, and even Google only shows such an offer in 2018.
