A buyer returned item saying it was not as described.
I guess that means I have to pay for outward and return shipping?

The buyer paid for any postage you had on your listing so you are returning their payment for this, you refund all the money the buyer initially paid.
Yes, for item not as described, if you want the item sending back to you before you refund, then you have to send and pay for that fully tracked return postage.
