If the buyer has opened a case on ebay the refund will be issued automatically when you accept it. If it's a return you will get the item back first, if you want it back.

The buyer has to open a case to return the item, what is the reason for the return/refund?

That extra information makes a lot of difference as the buyer has nothing to return so can't open a returns case.
You need to cancel the sale to give the buyer their refund. If the funds haven't cleared then eBay will use the funding source you set up for managed payments to make the refund, and will then release the cleared funds to you.
You will get all but 30p of your fees back but as you made a listing error you will also get a selling defect.

You can cancel even if the funds have cleared. You just go to the drop down options next to the item in your sold list.
If you don't cancel the buyer can open a case for item not received, but that will delay their refund and increase the chance for poor feedback for you.
It doesn't matter if it was a genuine error or not, you still made an error and inconvenienced the buyer.
