Good morning from the UK,
Need some assistance. I sold a dress on ebay unfortunately I didn't see small damage on the front. the buyer received it notified me they want to return it. they have sent back the item but not via ebay process, I need to issue a refund for the dress and postage but when I go to refunds it will not let me add the two amounts together, is there a way around this.
Appreciate your help.

If you cancel the transaction the buyer should get the original postage back from ebay. On your orders page click on the dropdown menu arrow alongside the item and select Cancel order > Buyer asked to cancel. The item refund will be issued in the process.
if you also want to refund the return postage you'd have to send that as a payment, eg. via Paypal.

If you mean they did not use the ebay label which you sent, you can only refund the amount they originally paid. To refund any extra amount will need to be made using paypal or bank transfer.

Just to add to the correct information given, if this was a Simple Delivery transaction you will not see the original postage amount paid, to refund.
eBay will refund this to the buyer, and worth sending the buyer a message explaining this, and that the postage refund can take up to 30 days to reach them.
