I sold a working fridge last month. The buyer contacted me to say it would not turn on, and sent photos.
I immediately sent a full refund (on 26th Feb) and today (7th March) the buyer just opened a return request, and eBay are asking me to respond and refund, when I already did

If you had refunded through a case then buyer would not have been able to open a return case. It does seem unlikely but you could be forced to refund again.
It does pay to follow correct procedures.

Like John says, as you refunded via a cancellation the buyer shouldn't be able to open a return, and of course you cannot refund again. It's not even possible to refund twice.
Check with the buyer that the cancellation and refund have gone through. If so ask them to close the return case. If you want the fridge back you will have to send them the code to a postage label.
