Years ago, I issued a refund first and did not get the item back, but I had a query email from eBay asking how the refund etc went. I replied to the email that I had refunded as asked but never got the item back. eBay contacted the Buyer and gave them a time limit to return the item to me.
The item was returned to me a couple of days later.
I'm not sure if eBay still helps in this situation.

eBay will give the seller no support, even with tracking proof of delivery.
All you can do is ask your buyer to return either the item or the refund. Many, regrettably, will just take the opportunity to keep the item for nothing.

You should never refund until after the item has been returned.
Contact the buyer once more. Be pleasant and polite and explain that they should not expect to have a refund and keep the item, and offer them a way of returning the item to you - a label or a payment for postage.
