If I click on issue a full refund. The refund only shows the amount I got for the item and not the postage cost.

Will eBay refund the postage? I've not used a postage label.

I'm just wondering why it doesn't show a full refund including postage and fees

You can only refund what you got, any amounts paid to ebay are refunded by ebay.
