find the item in my ebay and click on leave feedback

you may have to click on more actions to find it

There are many routes to leave feedback.

Find the item in your ebay summary, next to it will be an option to leave feedback.

Or, to the left of your My eBay page are some clickable text links, one of them is to leave feedback.

Or. click on the Feedback Forum text link to the left of this page.
